Our searchable Provider Directory includes information regarding primary care providers, specialists, ancillary providers, hospitals, urgent care centers, and pharmacies that participate with Jai Medical Systems’ provider network. Please note that this information is subject to change.
How Provider Information is Verified
The information provided in our searchable Provider Directory is reported and validated by the participating provider at least annually, or earlier if requested by a State or Federal agency.
In addition, Jai Medical Systems will independently verify the following information at least once every three (3) years and prior posting it online:
- Provider Specialty
- Board Certification
- Hospital Affiliation
- Hospital Accreditation
If a provider notifies us of a change to any of the information presented, we will update our online searchable provider directory within 30 calendar days of notification.

To ensure that the information in our directory is accurate at all times, network providers are responsible for maintaining the accuracy of their listings.
If you notice any errors in your provider listing or need to report change to your listing, please contact our Provider Relations Department at 1-888-JAI-1999 (1-888-524-1999). Changes may also be reported online by submitting an Update Your Information Form.
